Indonesia's metabolic health crisis affects an estimated 93 million adults who are overweight or obese, yet most never reach a doctor. Sirka, a startup backed by ACV Capital, is expanding from a wellness app into an integrated care provider, opening physical clinics and deploying AI that has nearly doubled patient conversion rates while cutting physician time per patient by more than half.

Founded in 2021, Sirka began as a diet coaching and consultation app. It has since partnered with healthcare clinics and opened two physical locations in Greater Jakarta focused on weight management, digestive health, diabetes care, and chronic disease management. Three additional clinics are planned. The company was also named in the Biotechnology & Healthcare category of Forbes Asia 100 to Watch 2026.

From single touchpoint to full care cycle

Sirka now offers physician-led care, diagnostics, pharmacy services, and structured health programmes. This means patients can move from initial consultation through diagnosis, medication, and ongoing monitoring within one system. The broader service range has diversified revenue and pushed the business to profitability.

"Most people who need metabolic care in Indonesia never reach the doctor. They drop out before the first conversation starts," said Rifanditto Adhikara, Co-Founder and CEO of Sirka.

AI cuts wait times and lifts conversion

AI is reshaping the patient intake process. Sirka's AI Receptionist handles intake, triage, and risk routing. A separate Doctor AI reviews patient information and prepares a structured pre-analysis before the physician steps in. Doctors retain full control over diagnosis and treatment decisions.

Before the AI tools, patients waited a median of roughly 26 minutes for a consultation, with most of that time consumed by manual intake and routing. After deployment, median wait times dropped to about three minutes. The shorter wait produced an unexpected result: conversion among newly acquired patients jumped from 33% to 60%. Sirka attributes the increase to patients reaching care while their intent was still high, rather than abandoning the process.

Physician time per patient fell from 18 minutes to eight, a reduction of approximately 55%. Patient information and medical records are now analysed in advance, so doctors spend less time on pre-consultation review. During the pilot, the same clinical team handled roughly 947 patients without adding headcount.

"The broader shift is that care begins to scale with software and data rather than linearly with headcount," Ditto said.

Consumer appetite for AI in healthcare is growing. A 2026 BCG study of more than 13,000 consumers across 15 countries found that nearly 60% already use generative AI for personal health, with adoption highest in emerging markets. The same research showed consumers still want human involvement as AI for Healthcare becomes more common.

Sirka's model shows that AI-driven intake and pre-analysis can reduce the administrative load on clinicians without replacing clinical judgment. For healthcare teams facing capacity constraints, the combination of shorter wait times, higher patient conversion, and reduced physician minutes per consultation offers a concrete template for scaling care delivery.
