AI agent for midwifes

High-Risk Referral Coordination Agent

Timely consult for every risk factor and a documented plan

High-Risk Referral Coordination Agent: what goes in, what the agent does and what you get

What it does

When a risk factor appears in pregnancy, such as high blood pressure or a growth concern, the midwife must consult or refer according to guidelines. This agent watches the chart for risk factors listed in the practice's consultation guideline and prepares a referral summary. It tracks the consult appointment and the report. If the appointment is not booked within the set days, it reminds the midwife. After the consult, it checks that the plan agreed with the obstetrician is in the chart and that the client knows the next steps. If the plan is missing, it requests the report and reminds the midwife. The midwife approves every referral and transfer of care. Edge case: urgent findings, like severe blood pressure, start the urgent pathway instead of a routine referral.

How it works

Follow the arrows from top to bottom. The orange dashed arrow is the loop: when a check fails, the agent goes back and tries again.

An example run

What happensAt 36 weeks a client's blood pressure read 148 over 96 twice. The agent matched it to the consult category in the practice guideline and drafted a referral summary with readings and labs. The midwife sent the referral. The consult happened on May 3, but a week later the check found no shared plan in the chart. The agent asked the obstetric office for the report, which arrived, and the plan was documented.
