AI agent for nurses
Patient Teach-Back Documentation Agent
Each key teaching point is confirmed understood and documented before discharge.
What it does
Patient education is often documented as given, but nobody confirms that the patient understood. For each patient's teaching plan, the agent lists the topics due, such as medicines, wound care or warning signs, and builds teach-back questions in plain language. After the nurse teaches, the nurse records the patient's answers and the agent compares them with the key points. If the patient cannot repeat a key step, it builds a simpler explanation with a different example and prompts the nurse to ask again. It loops until each key point is understood or the nurse flags it for follow-up. The nurse approves the questions and the final record. Edge case: a patient with a language barrier is given materials in their language and an interpreter prompt.
How it works
Follow the arrows from top to bottom. The orange dashed arrow is the loop: when a check fails, the agent goes back and tries again.
