What it does
Patients on chronic medicines often stop quietly, and the pharmacy only learns when something goes wrong. Each week this agent reads fill data and finds patients with late refills. It checks the likely reason: cost, side effects, supply problems, or a change by the prescriber. It drafts a short message and options to match the reason, such as a cheaper alternative, a 90-day supply or a synchronized fill date. The pharmacist approves each message. The agent tracks replies. If the patient does not respond, or if cost is the reason, it checks for a cheaper option before the pharmacist calls. Edge case: the patient was hospitalized, so the agent holds outreach and tells the pharmacist.

How it decides
It marks a refill late at 7 days beyond the expected date and picks the outreach by reason code. Cost cases get a price check first.
