What it does
A new opioid prescription is one of the riskiest items a pharmacist reviews, and counting doses by hand is easy to get wrong. This agent runs when a prescription arrives. It calculates the daily dose in morphine equivalents, adds other opioids on the profile, checks for risky combinations such as benzodiazepines, and checks whether the patient has naloxone. If a threshold is crossed, it drafts a message to the prescriber with the numbers and options, and it tracks the reply. If the prescriber changes the plan, it recalculates the dose and checks again before filling. The pharmacist approves every message and the final fill. Edge case: the patient is switching products, so the agent adds the old dose to avoid underestimating risk.

How it decides
It flags a prescription at or above the set daily dose, with a benzodiazepine, or with no naloxone, and re-runs everything on any change.
- Flag at or above 50 morphine milligram equivalents per day
- Flag any benzodiazepine on the profile
- Offer a naloxone discussion if none is on the profile
- Count overlapping fills when switching products
