What it does
Discharge can be announced with a day's notice and the home is not ready: no bed rail, no commode, no home nurse booked. The agent reads the discharge instructions and care plan, then lists the equipment, supplies, services and home changes needed, such as a hospital bed, a ramp, wound dressings or visiting nurse. It checks what is arranged and what is not and drafts calls or messages to suppliers and agencies. Before discharge day it rechecks each item, confirming delivery times and service start dates. Where an item is late, it finds an alternative or tells the caregiver and the discharge nurse. The caregiver approves every order or message. Edge case: if the home has stairs and the plan needs a ground-floor bed, the agent raises it at once.
