What it does

Some medicines are more harmful as people age, such as sleep aids, certain anticholinergics and long-acting sulfonylureas. This agent scans the profiles of patients over a set age and finds medicines on a high-risk list. For each, it checks the stated reason, the length of use and the possible alternatives. It drafts a short review for the prescriber with the concern, the evidence source and one or two options. The pharmacist approves it. The agent then tracks the answer and records the outcome. If the prescriber keeps the medicine, it notes the reason and sets the next review date. Edge case: the same patient has three flagged medicines, so the agent groups them into one message instead of three.
