A yearly medication review means gathering medicines, diagnoses and labs from several places before any thinking begins. This agent starts on each patient's schedule. It compiles the current medicines and conditions, checks for duplication, gaps in therapy such as no statin after a heart event, and cost options. It drafts a review document with the findings in priority order and prepares prescriber questions. The pharmacist approves the review and messages. The agent tracks prescriber replies. Each answer is read and the profile is updated before the next review date is set. Edge case: two prescribers gave conflicting plans, so the agent lists both and asks the pharmacist to choose how to raise it.
