No permit reaches approval with a missing or out-of-date safety check.
What it does
Hot work and confined space permits are signed under time pressure, and a missing gas test or isolation can be missed. For each permit request the agent reads the form and the work details, then checks that the required isolations are recorded, the gas test is in date and in range, the people named are trained and certified, and no other job in the area conflicts, such as welding near a fuel line. It lists each gap and sends it back to the requester. After corrections, it checks the permit again and only passes it when every item is clear. A person must approve before work begins, and the agent can never issue a permit. Edge case: a gas test older than the allowed time forces a fresh test.
