AI agent for medical records clerks

Referral Packet Readiness Agent

Every referral is administratively complete, with documents for the right patient.

Referral Packet Readiness Agent: what goes in, what the agent does and what you get

What it does

Specialist clinics often receive referrals without required documents or with patient identifiers that do not match. When a referral or attachment arrives, this agent first checks that every document belongs to the same patient, comparing name, date of birth and ID. Any document that does not match is quarantined, and the coordinator is alerted. It then matches the remaining documents to the clinic's administrative checklist. For each missing item it creates a follow-up task, and it rechecks completeness when new documents arrive. The referral coordinator accepts or rejects the referral, and the agent never makes clinical decisions or contacts patients. Edge case: an imaging report has a different date of birth, so it is quarantined even though the name matches.

How it works

Follow the arrows from top to bottom. The orange dashed arrow is the loop: when a check fails, the agent goes back and tries again.

How it decides

It checks the next gap on the checklist and identifier match for every document; mismatches are quarantined.

  • Identifier match: name, date of birth and ID must agree.
  • Quarantine: any mismatch.
  • Complete: all checklist items present.

Make it yours

Every agent is a starting point. You choose these settings for your own situation.

  • Checklist per clinic or referral type
  • Identifiers that must match (default name, date of birth and record ID)
  • Hours before an incomplete referral is flagged (default 48)
  • Who receives quarantine alerts
  • Exception list format

What keeps you in control

It always asks you first

  • Patient contact
  • Clinical prioritization
  • Accepting or rejecting referrals

Hard limits

  • No clinical decisions.
  • Mismatched documents are never filed to the record.

It stops when

  • Done: packet complete.
  • Blocked: identity mismatch unresolved.

An example run

What happensOn June 3 a cardiology referral arrives with 4 attachments. An imaging report shows a date of birth of 1958, while the referral says 1965. The identity check fails, so the agent quarantines the report and alerts the coordinator. The checklist also shows no medication list, so it creates a follow-up task. Both arrive June 5; the agent rechecks and the coordinator accepts the referral.
