Healthcare Must Govern AI Systems That Act, Not Just Advise

Healthcare is moving from AI tools that support clinical decisions to systems that can execute them autonomously. That shift carries new risks that require operational governance, not just policy documents.

Clinicians and staff already face mounting pressure from documentation demands, staffing shortages, and workflows stretched thin. AI that reduces administrative burden has clear appeal. But agentic AI-systems that initiate actions, move information across workflows, and complete tasks with minimal human intervention-operates in a different risk category.

Where Hidden Risks Live

Healthcare has no purely administrative workflow. A scheduling decision that looks routine on paper can carry serious clinical consequences.

Consider a patient referred for an ultrasound due to new leg swelling. An AI agent schedules it for next week. The system completed the task. But if that swelling stems from deep vein thrombosis, and the clot reaches the lungs before the test, the patient ends up in the ICU with pulmonary embolism. This is not a hypothetical edge case-it is the kind of outcome clinicians worry about because context matters.

The same logic applies to inbox management and medication refill workflows. Most messages are routine. Most refill requests are appropriate. But hidden in those high-volume tasks are the exceptions that matter. A portal message describing minor symptoms may signal silent myocardial infarction in a diabetic patient. A refill request may seem automatic until you realize the patient has not had their renal function checked and the medication is no longer safe.

If an autonomous system cannot reliably recognize when a workflow crosses from administrative to clinical, it creates risks quickly.

Accountability Becomes Unclear

When an AI agent makes a decision with downstream consequences, who owns the outcome? The ordering physician? The staff member who would have scheduled it traditionally? The health system that approved the workflow? The vendor? The team that configured the agent?

More critically: did any human being see the decision at the point when intervention still mattered?

Governance Must Be Operational

Healthcare already knows how to work in high-risk environments. The same principles apply to agentic AI.

At the start, these systems should be tightly provisioned. They should do only what they are explicitly authorized to do, under clearly defined conditions, with human oversight built in. A human in the loop is not a sign the technology failed-it is how healthcare manages responsibility when consequences are real.

As organizations gain confidence with specific workflows, some may reduce direct human review. But that requires the right controls:

  • Defined identity for each AI agent
  • Tightly constrained permissions
  • Clear rules about when it can act and what it can access
  • Observable behavior and detectable deviations
  • Attributable actions with audit trails
  • Ability to pause, override, or revoke access

Govern AI agents like participants in the care environment, not background software. The question is not just "What can this agent do?" but "Under what conditions should we trust it to act?"

The Standard Remains Unchanged

Friction in workflows is frustrating. Unsafe automation is worse. Agentic AI will succeed in healthcare not because it moves fast, but because it moves safely, predictably, and accountably within the realities of patient care.

AI will play a larger role in healthcare. When these systems begin acting rather than advising, they become a new operational actor in the system. Anything that can act in healthcare must be governed accordingly.
