Many healthcare organizations operate with outdated vendor lists while AI-related threats-including synthetic data misuse, training data leakage, and adversarial attacks-go unreported by vendors. The problem extends beyond new deployments. AI capabilities have proliferated across clinical decision support systems, electronic health records, remote monitoring devices, and administrative tools without formal oversight at many institutions.
Why Traditional Risk Management Falls Short

Phase 3: Implementation, Integration, and Training
AI implementations require validation beyond traditional software deployment. Organizations must conduct AI-specific threat modeling addressing behavioral vulnerabilities such as prompt injection, data poisoning, and model manipulation.
Technical integration proceeds through sandbox testing, security validation, and clinical validation before production rollout. Organizations must verify that threat model controls function correctly, encryption and access controls are in place, and human override capabilities work. Clinical validation must confirm that AI outputs are treated as untrusted until validated by humans.
Organizations must also conduct or update Privacy Impact Assessments, establish AI-specific incident response playbooks, and provide role-specific user training covering AI limitations, error recognition, and override procedures before granting production access.
Phase 4: Ongoing Monitoring and Performance Management
This phase demands the most intensive resource commitment. AI systems require continuous monitoring because models drift as input data changes, performance degrades gradually, and vendor updates introduce risks that standard change management cannot address.
Key monitoring activities include tracking model accuracy, false positive and negative rates, and user override patterns. Organizations must detect model drift and concept drift against defined thresholds and monitor AI performance across demographic subgroups to identify discriminatory outcomes.
Vendor update and patch management requires a structured process: receiving and assessing update notifications, deploying to a sandbox environment first, validating that security settings were not reset, and conducting post-deployment monitoring before full production approval.
Phase 5: Incident Response and Recovery
Traditional IT incident response procedures are insufficient for AI failures. AI incidents can manifest as gradual degradation rather than catastrophic failure and may involve corrupted training data, accumulated model drift, or emergent behaviors.
Organizations must prepare for AI-specific scenarios including security breaches affecting training data, model performance failures, bias events producing discriminatory outputs, adversarial attacks, and model hallucinations. Effective response requires pre-established frameworks covering incident classification, vendor coordination with contractually defined notification timeframes, immediate containment actions, and forensic investigation.
Recovery requires validating that model performance, data integrity, and security controls have been fully rehabilitated before returning to normal operations. Organizations may need to rollback to previously validated model versions and conduct abbreviated revalidation in the current environment.
Phase 6: End-of-Life and Transition Management
AI systems present distinct end-of-life challenges. Models may depend on external services deprecated without the primary vendor's control. Organizational data embedded in model weights requires specialized destruction beyond standard data deletion. Replacing one AI model with another may not maintain equivalent clinical performance without comprehensive revalidation.
Proactive planning must begin during initial contracting by securing vendor notification requirements, data extraction rights, and secure destruction procedures. Upon receiving an end-of-life notification, organizations must assess operational, clinical, cybersecurity, and regulatory impact and decide whether to replace or discontinue the system.
Data management is critical. Organizations must inventory and classify all associated data-including training datasets, audit trails, and user interaction logs-then extract data in interoperable formats, migrate or archive per retention policies, and obtain vendor-certified secure destruction per NIST standards.
Building Governance and Accountability
Healthcare organizations should establish AI governance structures aligned with their size and complexity. Clear accountability for oversight, security attestations, risk categorization, and approval processes must be defined.
Shared responsibility models with vendors require enforcing contractual transparency, requiring advance notification of changes, and conducting joint validation activities. Procurement workflows must identify the presence of AI early in the acquisition process and ensure comprehensive vetting before deployment.
Greater vendor transparency is essential, particularly around model training data, potential biases, and system dependencies. Organizations should maintain active inventories and use dynamic risk profiling alongside scalable due diligence tools to surface hidden dependencies.
