Controlled Substance Refill Monitoring Agent

Every controlled refill is reviewed with all required checks current

Controlled Substance Refill Monitoring Agent: what goes in, what the agent does and what you get

What it does

Prescribing stimulants, benzodiazepines or opioids comes with rules: drug monitoring database checks, treatment agreements, urine screens and refill intervals. Keeping track by hand is error-prone. This agent reviews each refill request for a controlled drug. It checks days since the last fill, the state monitoring database report, the agreement date and the date of the last screen. It flags early refills, overlapping prescribers and missing checks, and prepares a short summary for the prescriber. If a required check is missing, it schedules it or reminds the patient before the refill is due, then checks again. The prescriber makes every refill decision and any change to dose or drug. Edge case: a lost-prescription request is never processed automatically; it goes to the prescriber with the full fill history.

How it works

Follow the arrows from top to bottom. The orange dashed arrow is the loop: when a check fails, the agent goes back and tries again.

How it decides

Each refill is tested against interval, monitoring check, agreement and screen rules. Any failed rule goes to the prescriber with the evidence.

  • Refills more than 3 days early are flagged
  • More than one prescriber in 90 days is flagged
  • Agreements older than 12 months need renewal

Make it yours

Every agent is a starting point. You choose these settings for your own situation.

  • Drug classes in scope
  • Early refill tolerance
  • Agreement renewal interval
  • Screen frequency

What keeps you in control

It always asks you first

  • Every refill decision
  • Any change to dose or drug

Hard limits

  • The agent never sends prescriptions
  • No refill is approved by default

It stops when

  • Done: decision documented
  • Stop: monitoring database unavailable, prescriber decides with a note

An example run

What happensA refill request for an adult ADHD stimulant arrived on November 4, 24 days after the last 30-day fill. The agent pulled the state monitoring report and found the treatment agreement was 13 months old, past the 12-month rule. It went back and searched the chart for a renewal, found none, and flagged it. It wrote a one-page summary. The prescriber approved the refill on the due date and scheduled the agreement renewal.
