What it does

Therapy plans of care have end dates, visit counts and physician signature rules, and payers deny visits outside them. Each day this agent lists plans within 14 days or 2 visits of their limit. It pulls the latest outcome scores and goal status. If goals are already met, it suggests discharge planning instead of recertification. Otherwise it drafts a progress summary for re-evaluation. The therapist completes and signs the re-evaluation and owns all clinical content. The agent then checks that the referring provider's signature and the new authorization are in place before more visits are booked. If either is missing, it sends the request and rechecks daily. No visits are booked past the limit without both. Edge case: a patient who meets goals early gets discharge planning rather than a new plan.
