If the patient has not confirmed 48 hours before, it calls again and arranges a ride. If no visit is booked, it escalates to the clinician. The clinician approves the booking choices and any contact outside the normal list. Edge case: the patient has no phone, so the agent uses the discharge contact or the care team listed in the chart.
